Understanding Eyelid Surgery: Upper and Lower Blepharoplasty

Which Surgery Do You Need?

If your eyes feel heavier or less defined than they used to, the cause is usually structural rather than cosmetic. Skin laxity, shifting fat, and loss of support around the eyelids all contribute to a tired or closed appearance. Eyelid surgery is designed to address these changes at their source, but knowing whether upper or lower blepharoplasty is right depends on where those changes are happening.

What Blepharoplasty Is Designed to Address

Eyelid surgery, also known as blepharoplasty, focuses on removing or repositioning excess skin and fat around the eyes. The goal is not to change the shape of your eyes or create a dramatic look. It is to restore clarity, openness, and balance so your expression feels natural and refreshed.

There are two main types of eyelid surgery:

  • Upper blepharoplasty
  • Lower blepharoplasty

Each treats a different concern, and understanding the difference helps ensure the right approach.

Upper Blepharoplasty and Hooded Eyes

Upper blepharoplasty addresses excess skin and tissue above the eyes. Over time, this skin can stretch and fold, creating what many people describe as hooded eyes or hooded eyelids. This heaviness can make the eyes appear smaller, less alert, or difficult to define with makeup.

People often consider upper eyelid surgery when they notice:

  • Hooded eyelids that rest on the lash line
  • A heavy or tired appearance in photos
  • Difficulty applying or seeing eye makeup
  • Reduced openness or peripheral vision

Upper eyelid surgery removes the excess skin causing this heaviness, allowing the eyes to look more open while preserving natural expression. Patients often describe the result as looking more awake rather than “done.”

Lower Blepharoplasty and Under Eye Changes

Lower blepharoplasty focuses on the area beneath the eyes. This is where under eye bags, puffiness, and contour irregularities tend to develop.

Lower eyelid surgery may be appropriate for concerns such as:

  • Persistent under eye bags that do not improve with sleep
  • Puffiness caused by shifting fat pads
  • Shadowing or uneven contours beneath the eyes
  • A tired or worn look despite good skin care

Modern lower eyelid surgery is rarely about removing fat alone. In many cases, fat is repositioned or supported to avoid a hollow or tight appearance. The goal is a smooth transition between the lower eyelid and the cheek.

Do You Need Upper or Lower Eyelid Surgery, or Both?

Many patients experience changes in both the upper and lower eyelids at the same time. Treating only one area while leaving the other untreated can sometimes create imbalance.

When hooded eyes and under eye changes are both present, a combined approach often provides the most natural result. This is sometimes referred to as total eye rejuvenation, where eyelid surgery is planned as a complete system rather than a single fix.

Can Eyelid Surgery Be Combined With Other Procedures?

Yes. Blepharoplasty is often combined with fat grafting or other facial procedures when volume loss contributes to a tired appearance. Carefully restoring volume around the eyes can soften hollows and improve overall balance.

Not everyone needs combination procedures. The key is choosing an approach that fits your anatomy and goals rather than adding treatments unnecessarily.

Recovery

Recovery from eyelid surgery is generally manageable for most patients.

What to expect:

  • Swelling and bruising for one to two weeks
  • Gradual improvement as tissues settle
  • Final results that continue to refine over several months

Many patients feel comfortable returning to normal activities relatively quickly, especially after upper eyelid surgery.

How to Know Which Surgery Is Right for You

A helpful way to think about it:

  • Hooded eyes or heaviness above the eyes often point toward upper eyelid surgery.
  • Under eye bags or puffiness suggest lower eyelid surgery.
  • Concerns affecting both areas may benefit from a combined approach.

An in-person consultation is essential. The eyelid area is complex, and subtle anatomical differences significantly influence surgical planning.

The Takeaway

Eyelid surgery is not about changing how you look. It is about removing the structural changes that make your eyes appear tired or closed.

The best eyelid surgery results are subtle. People notice that you look rested and refreshed, but they cannot pinpoint why. That is the sign of thoughtful planning and a balanced approach.
